wSw believes returning to a familiar environment, such as a patient’s home, assists in achieving a speedier recovery.
wSw has developed a post-acute care in-home program designed to support patients with their transition from hospital to the home.
wSw designed its processes around ensuring that care and support from occupational therapists, nursing staff, care/support staff, physiotherapists, geriatricians, specialists and any other required health professional, are provided. We work in partnership with the discharging hospital and the client’s GP, with wSw staff maintaining management of each and every case until full recovery objectives are achieved.
